Carbon Valley Eye Care has been proudly serving the Carbon Valley community and neighboring towns since 2005 (we used to be located next to the Firestone Safeway!)

 

Our mission is to deliver the highest level of care to every patient. Our doctors provide compassionate and experienced care using advanced diagnostic equipment. 

meet our doctors

dr mike koditek

Dr. Koditek graduated from the University of Colorado before earning his Doctor of Optometry degree (with honors) in 2011 from Pacific University College of Optometry. After beginning his career in Loveland, he transitioned to Carbon Valley Eye Care in 2014.

 

Dr. Koditek received intensive training in medical eye care during his six-month internship at the American Lake Veterans Hospital in Tacoma, Washington. He subsequently achieved Fellowship in the American Academy of Optometry in 2013. He provides medical and routine eye care, with an interest and expertise in managing dry eye and fitting specialty contact lenses.

 

He is a member of the Colorado and American Optometric Associations and maintains Fellowship in the American Academy of Optometry. He is also a member of the Carbon Valley Chamber of Commerce.

 

Dr. Koditek is married to Kim, and they have two children. Outside of the office, he enjoys spending time with his family, playing ice hockey, and fly fishing.

dr taylor kiyota jackson

Dr. Taylor Kiyota Jackson was born and raised in Louisville and Fort Lupton, Colorado. She earned her Bachelor of Science in Microbiology from the University of Washington before graduating from the Southern California College of Optometry summa cum laude.

 

Dr. Kiyota joined Carbon Valley Eye Care in 2020. She provides routine and medical eye care with a special interests in routine care, pediatrics and dry eye. Her grandfather, Dr. Michael Fukai, is a retired Optometrist, who provided exceptional eye care in Louisville for 30 years. She strives to continue this legacy by providing a personable bedside manner and high standard of care.

 

Dr. Kiyota is married to her high school sweetheart, Victor, and they enjoy spending time their daughter and two dogs. She's a fifth generation Coloradan. Her family owns Kiyota Greenhouses, located in Fort Lupton, and she enjoys providing eye care so close to home.

dr matt winkler

Dr. Winkler graduated from the State University of New York at Geneseo with a Bachelors of Science degree in Biology in 2005. After developing a particular interest in vision, he attended Nova Southeastern University College of Optometry in south Florida. During his time there he was awarded a spot in the Beta Sigma Kappa National Honorary Society and was chosen by his peers as class representative and treasurer, serving on the local Optometry board.
 

He graduated with honors in 2011 with a Doctorate of Optometry, subsequently training at the Wm Jennings Bryan Dorn Veterans Medical Center in Columbia, South Carolina. Training here focused extensively on the diagnosis and management of ocular disease.
 

Afterwards, Dr. Winkler moved back to the lower Hudson Valley of New York, where he grew up, and began serving his local community. In 2020 he furthered his education, becoming a board certified Optometrist attaining Diplomate status at the American Board of Optometry.
 

Outside of work, Dr. Winkler is an avid sports enthusiast who enjoys comedy shows, hiking, cooking and spending quality time with his family and friends. He is looking forward to helping all in the local Colorado community with their eye care needs.
