dr mike koditek
Dr. Koditek graduated from the University of Colorado before earning his Doctor of Optometry degree (with honors) in 2011 from Pacific University College of Optometry. After beginning his career in Loveland, he transitioned to Carbon Valley Eye Care in 2014.
Dr. Koditek received intensive training in medical eye care during his six-month internship at the American Lake Veterans Hospital in Tacoma, Washington. He subsequently achieved Fellowship in the American Academy of Optometry in 2013. He provides medical and routine eye care, with an interest and expertise in managing dry eye and fitting specialty contact lenses.
He is a member of the Colorado and American Optometric Associations and maintains Fellowship in the American Academy of Optometry. He is also a member of the Carbon Valley Chamber of Commerce.
Dr. Koditek is married to Kim, and they have two children. Outside of the office, he enjoys spending time with his family, playing ice hockey, and fly fishing.

dr taylor kiyota jackson
Dr. Taylor Kiyota Jackson was born and raised in Louisville and Fort Lupton, Colorado. She earned her Bachelor of Science in Microbiology from the University of Washington before graduating from the Southern California College of Optometry summa cum laude.
Dr. Kiyota joined Carbon Valley Eye Care in 2020. She provides routine and medical eye care with a special interests in routine care, pediatrics and dry eye. Her grandfather, Dr. Michael Fukai, is a retired Optometrist, who provided exceptional eye care in Louisville for 30 years. She strives to continue this legacy by providing a personable bedside manner and high standard of care.
Dr. Kiyota is married to her high school sweetheart, Victor, and they enjoy spending time their three kids. She's a fifth generation Coloradan. Her family owns Kiyota Greenhouses, located in Fort Lupton, and she enjoys providing eye care so close to home.

dr michael bagley
Dr. Bagley is a Colorado native, born and raised in Lakewood. After graduating from Pacific University College of Optometry in 2017, he joined the Army and served until 2020. After his military separation, he moved back to Colorado and is excited to be working in his home state. He began his time at Carbon Valley Eye Care in 2025.
Dr. Bagley is passionate about providing quality vision care to everyone. He strives to be inclusive of all individuals regardless of their background. His areas of specialty include pediatrics and traumatic brain injury care. For 3 years, he worked almost exclusively with children. Additionally, his time in the Army provided training in regard to traumatic brain injury and its effects on a person’s vision.
He and his wife, Kelly, have two amazing cats. They love to travel and explore together, including hiking, kayaking, and discovering new adventures.

dr ryan dugan
Dr. Dugan graduated with honors from the University of Oregon with a degree in Human Physiology before earning his Doctor of Optometry degree with distinction from Pacific University College of Optometry. While at Pacific University, he was an active member of the Oregon Optometric Physicians Association and Beta Sigma Kappa Honor Society and was named OOPA Student of the Year in 2017 for his advocacy for the profession of optometry.
Following optometry school, Dr. Dugan completed a residency at the Southern Arizona VA Health Care System, where he received advanced training in ocular disease and low vision rehabilitation. He has experience in private practice, commercial, and hospital settings and provides both routine and medical eye care, including the diagnosis and management of conditions such as glaucoma, cataracts, and macular degeneration.
Dr. Dugan has also participated in international service trips through Volunteer Optometric Services to Humanity, providing eye care to underserved populations in Africa and Asia. He is passionate about providing high-quality eye care while helping patients better understand their eye health and diagnoses.
Outside of the office, Dr. Dugan enjoys spending time outdoors and with friends and family.

Meet our managers

Office Manager
Christina
Christina was born and raised in Colorado. She brings over 20 years of optical experience to Carbon Valley Eye Care, having worked in all aspects of an optometry office throughout her career. She uses all this knowledge to help our office run smoothly. Christina is also an ABO certified optician so you may sometimes see her helping patients in our optical department.
Outside of work, you may see Christina at local youth sporting events cheering on her three children along with her husband, Jason. She also enjoys baking and traveling with her family.

Technician Manager
Cassie
Cassie is a Colorado native and has been part of the Carbon Valley Eye Care team
since April 2024. She earned her Certified Paraoptometric (CPO) credential in the fall of
2025 and is dedicated to providing exceptional patient care while supporting the entire technician team.
Outside the office, Cassie enjoys spending time with her husband and son. She also
loves reading and practicing yoga.

Patient Coordinator Manager
Chelsey
Chelsey is a Frederick native who brings a warm, people-focused approach to patient
care. She is currently pursuing a degree in medical coding to grow her healthcare
expertise.
Outside of work, she enjoys crafting, interior design, event planning, and is a certified
florist. Chelsey stays busy adventuring around Colorado with her wife and two boys,
traveling, rock climbing, exploring breweries, and enjoying the outdoors.

Optical Manager
Gabby
Gabriella brings over eight years of experience as an optician and has been ABO
certified since 2021. She became a Certified Paraoptometric Assistant (CPOA) in 2023
and has been a dedicated member of the Carbon Valley Eye Care team for six years,
contributing her expertise in optical operations and patient care.
She genuinely loves helping patients see their best, combining strong technical skills
with a caring, supportive approach.
Outside of work, Gabriella enjoys spending time with her husband, Jonathon, creating
art, reading, and playing basketball.
