Our Story

Carbon Valley Eye Care has been proudly serving the Carbon Valley community and neighboring towns since 2005. Our mission is to deliver the highest level of care to every patient. Our doctors provide compassionate and experienced care using advanced diagnostic equipment. We provide medical eye care (including diabetic care and dry eye care), vision therapy, specialty contact lenses, and routine eye care for the whole family.

We offer the latest in cutting edge lens technology, high quality fashionable frames, children’s eyewear, sports eyewear, and safety glasses. We have accounts with frame and lens manufacturers across the country and locally, which allow us to quickly make the personalized eyewear you need, at competitive prices. We also have an in-house optical finishing lab run by our certified opticians.

Meet Our Optometrists

Dr. Michael Koditek
Optometrist

Dr. Koditek graduated from the University of Colorado before earning his Doctor of Optometry degree (with honors) in 2011 from Pacific University College of Optometry. After beginning his career in Loveland, he transitioned to Carbon Valley Eye Care in 2014.

Dr. Koditek received intensive training in medical eye care during his six-month internship at the American Lake Veterans Hospital in Tacoma, Washington. He subsequently achieved Fellowship in the American Academy of Optometry in 2013. He provides medical and routine eye care, with an interest and expertise in managing dry eye and fitting specialty contact lenses.

He is a member of the Colorado and American Optometric Associations and maintains Fellowship in the American Academy of Optometry. He is also a member of the Carbon Valley Chamber of Commerce.

Dr. Koditek is married to Kim, and they have two children. Outside of the office, he enjoys spending time with his family, playing ice hockey, and fly fishing.

Dr. Jeff Berger
optometrist

A Colorado native, Dr. Berger grew up primarily in Highlands Ranch and pursued his undergraduate degree at Colorado State University. He worked for several years as a research associate in biochemistry before attending optometry school at Northeastern State University Oklahoma College of Optometry in Tahlequah, OK. He graduated magna cum laude in 2009 and returned to Colorado to begin his optometric career.  

Dr. Berger started his own practice, Erie Family Eyecare, in 2010 and remained in that location until 2018. He merged with Dr. Koditek in order to offer a wider array of services in one practice.​

Dr. Berger provides primary care optometry including contact lenses and disease management. He has a specific interest in pediatric and developmental vision, visual rehabilitation, and vision therapy. He is a Fellow of the College of Optometrists in Vision Development.​

Outside of the office, Dr. Berger enjoys many physical activities including running, golf, and racquetball. He spends much of his free time with his wife, Leslie, and their two young children.

Dr. Taylor Kiyota
optometrist

Dr. Taylor Kiyota Jackson was born and raised in Louisville and Fort Lupton, Colorado. She earned her Bachelor of Science in Microbiology from the University of Washington before graduating from the Southern California College of Optometry summa cum laude. She graduated at the top of her class and received multiple clinical and academic awards throughout her education.

Dr. Kiyota joined Carbon Valley Eye Care in 2020. She provides routine and medical eye care with a special interest in medical optometry, dry eye management, low vision and pediatric care.

Her grandfather, Dr. Michael Fukai, is a retired Optometrist, who provided exceptional eye care in Louisville for 30 years. She strives to continue this legacy by providing a personable bedside manner and high standard of care.

Dr. Kiyota is married to her high school sweetheart, Victor, and they enjoy spending time outdoors with their two dogs. Her family owns Kiyota Greenhouses, located in Fort Lupton, and she enjoys providing eye care so close to home.

Carbon Valley Eye Care is proud to be a sustainable destination for all of its patients. With high-energy producing solar panels covering the roof of our center, we're able to produce more electricity than we use.

In addition, Carbon Valley Eye Care is an official drop site for Baush & Lomb's Terracyle program.
